Registration
Follow the four steps below to register your student at Cook Elementary:
1. Make sure your student lives in Cook Elementary boundaries.
Students must live in the Cook Elementary School boundaries to register. To check which school boundaries you are in click here.
2. Make sure your child's immunizations are up to date and you have proper documentation.
State law requires that each student's immunization records be up to date in order for the student to enter school. Immunization records must be turned into the school before your child will be allowed to start school. The immunization requirements are:
DPT 4 Shots (students need a 5th dose if they received the 4th dose before age 4)
ORAL POLIO 3 Doses
MMR 2 Shots
Hepatitis B 3 Doses (students born after July 1, 1993)
Hepatitis A 2 Doses (students born after July 1, 1996)
Chicken Pox (Students born after July 1, 1996)
3. Print and fill out the required forms.

4. Take the following items to the Cook Elementary Office to register:
Your child's original Birth Certificate
Your child's Immunization Record
Student Information Form
Proof of Residency Form
Guardianship Status Form
Homeless Assistance Act Form
Kindergarten Registration
Please click on the following link and carefully read the "Items Needed for Kindergarten Registration."
Please print and fill out the required forms (see step 3 above).
Take the completed forms with you and your student to the Kindergarten Roundup (first Thursday of March).
If you cannot attend the Kindergarten Roundup, please take the forms to the Cook Elementary Office as soon as possible.
